Florida Supervision Paperwork Includes:

Florida Psychologists Package:

This package is specifically tailored for Licensed Psychologists supervising other Psychologists and/or Registered Mental Health Counseling Interns (providing they have met the requirements to supervise them) in the state of Florida ONLY.

Category 1:

Florida Licensed Psychologists Supervising Psychologists 

  1. Ethical Codes – You will receive a copy of the APA ethics (also available online).

  2. Hours Log– Easy to update log to track clinical hours, face to face and supervision hours. This is for best practices so your supervisee will have proof of supervision if anything were to happen to you during the course of your supervision relationship.

  3. Supervision Contract– This is mandated for Florida psychologists before starting supervision. It covers the rules regarding state standards and expectations for supervision. Update your specific practice details in the highlighted areas.

  4. Supervision Psychologist Verification Form– This form must be completed when supervision requirements are met. You will need to send to the board once finished.

  5. Welcome Letter– Use this template to welcome your new supervisee and inform him/her about details regarding supervision.

Category 2:

Florida Licensed Psychologists Supervising 

Registered Mental Health Counseling Interns

  1. Core Competencies– These were written from an MFT perspective, but we include this in our packet as an overall measurement standard to rate your interns competencies. Currently, there are no other associations that have Core Competencies, so we use this as a guideline. They are also available online.

  2. Ethical Codes– You are receiving a copy of AMHCA and ACA ethics (also available online) since licensed psychologists are allowed to supervise MHCs.

  3. Hours Log– Easy to update log to track face to face and supervision hours. This is for best practices so your supervisee will have proof of supervision if anything were to happen to you during the course of your supervision relationship.

  4. New Supervisee Acceptance Letter– This is a template to use to inform the board that you are accepting a new intern (one who is not transferring from another supervisor) into your practice.

  5. Registered Intern Guide– Distribute this to your intern/s to help guide them along the licensure process.

  6. Supervisee Transfer Letter– This is a template to use to inform the board that you are accepting a transferred intern (one that has already received licensure supervision and is now working with you).

  7. Supervision Audit Sheet– This audit sheet is to help you stay organized and remain up to date with needed documentation on your intern/s.

  8. Supervision Contract– This covers the rules regarding state standards for supervision. Please assure you update your specific practice details in the highlighted areas.

  9. Supervision Experience Form– This is to be completed by the supervisor at the completion of supervision. The original will need to be sent to the board. Remember  your intern MUST have completed all their clinical hours, supervision hours, pass the state licensing exam and complete the pre-licensure courses. Interns must remain under supervision until they receive their license.

  10. Welcome Letter– Use this template to welcome your new supervisee and inform him/her about details regarding supervision.

Florida Mental Health Professionals Package:

This package is specifically created for LMFTs, LMHCs, and LCSWs supervising Registered Marriage and Family Therapist Interns, Registered Mental Health Counseling Interns, and/or Registered Clinical Social Worker Interns toward licensure in the state of Florida ONLY. 

  1. Core Competencies – These were written from a MFT perspective, but we include this in our packet as an overall measurement standard to rate your interns competencies. Currently, there are no other associations that have Core Competencies, so we use this as a guideline.   2. Ethical Codes – You are receiving a copy of AAMFT, NASW, ACA, and AMHCA ethics (also available online).

  3. Hours Log– Easy to update log to track face to face and supervision hours. This is for best practices so your supervisee will have proof of supervision if anything were to happen to you during the course of your supervision relationship.

  4. New Supervisee Acceptance Letter– This is a template to use to inform the board that you are accepting a new intern (one who is not transferring from another supervisor) into your practice.
